I could find a general explanation of how government organizations failed. I created this three-tier model after an extensive review of the literature and an in-depth study of four major government failures, the Auckland Development Authority Navy project, the Challenger accident and the Columbia accident. Much of the model is based on analysis of a failed Everest expedition into thin air. Seven years later I find the framework still useful for understanding how organizations fail.
